Patients

What should I bring on my 1st visit?

  1. Health History Form (please print and complete prior to appointment)
  2. HIPPA Form (please print and complete prior to appointment)
  3. Health Insurance Card
  4. Prior Test Results (x-rays, MRIs, medical reports, etc.)

What can I expect on my 1st visit?

Consultation & Examination - the doctor will review your personal medical history and ask you details about your current complaint(s). He will then perform a very thorough physical exam that includes an orthopedic, neurologic and chiropractic evaluation to assess joint, muscle and nerve function. Any need for additional testing (x-ray, MRI, EMG, etc.) will be discussed at this time.

If the doctor believes that he can help you with the conservative therapies offered in our office he will review your diagnosis and discuss the treatment recommendations for your particular condition. However, if the doctor believes you require treatment not available in our office he will also tell you and provide a referral to another appropriate physician.

Treatment Plan - If your case is accepted by the doctor, he will discuss your health-related goals and provide the recommended treatment to meet those goals. He will explain what is necessary to provide you with the best chance of quick recovery and prevention of long-term disability.

At Morris Spine & Sport Chiropractic our emphasis is on the following:

Lifestyle Advice - teaching our patients about proper posture, ergonomics, diet and stress management techniques to maintain a strong, healthy musculoskeletal system

Coordination of Care - working with medical physicians and other healthcare specialists to provide comprehensive care for our patients

Is chiropractic treatment safe?

As with any medical treatment there can be some risk. However, in general, chiropractic treatments carry an extremely low risk of serious complication.

Recent research demonstrates that spinal manipulation is one of the safest forms of treatment for neck and back pain...even safer than aspirin, acetominaphin or ibuprofen.

The doctor will review your treatment plan and the specific risks and alternatives for your specific condition and afford you the opportunity to ask any questions prior to beginning treatment.
